Most men know that regular exercise is good for them. Better heart health, lower stress, stronger muscles - the list goes on. But when it comes to male fertility, the story is a bit more nuanced. Exercise can genuinely support sperm health, but depending on how much you do and how you do it, it can also work against you.

Here's what the research actually says.

The upside: moderate exercise and sperm health

Regular, moderate exercise has a real and measurable positive effect on sperm. A 2024 review published in Frontiers in Physiology found that moderate exercise improves sperm count, motility (how well sperm swim), and morphology (their shape) - all three key things a fertility specialist assesses [1]. 

Exercise improves blood circulation, reduces oxidative stress (cellular damage that can harm sperm DNA), and helps keep hormones like testosterone, FSH (follicle-stimulating hormone), and LH (luteinising hormone) in better balance. 

Another study revealed that men carrying extra weight also tend to see sperm quality improvements once they become more active, largely because exercise helps with insulin sensitivity and weight management - both of which affect reproductive hormones [2].

A 2024 review noted that men who spent large amounts of time sitting - particularly watching television - had noticeably worse sperm concentration and count. Sedentary behaviour is not neutral [3]. 

Where it gets tricky: too much exercise

There's a point where exercise stops helping and starts causing harm on male fertility health. Researchers describe this as an "inverse U-shaped" relationship - benefits increase up to a point and then drop off [4]. At the extremes, very heavy training can suppress the same hormonal pathways that moderate exercise supports.

Men who train intensively – such as athletes or those who spend many hours a week in heavy training - can experience what's known as "exercise-hypogonadal male condition," where the body produces lower levels of testosterone due to the physical stress of over-training. High-endurance exercise also raises cortisol (a stress hormone), which essentially competes with testosterone's effects, according to the 2024 study [5]. 

Vigorous endurance training has also been linked to increased oxidative stress and DNA damage in sperm - the exact opposite of what moderate exercise does [6].

Cycling: a specific concern

Cycling deserves its own mention. It's a great form of cardiovascular exercise, but extended cycling - especially in tight-fitting clothing - raises scrotal temperature. Sperm production requires the testes to be slightly cooler than the rest of the body (about 2–6°C lower). When that temperature rises consistently, sperm production is affected. Research suggests that even a 1°C rise in scrotal temperature can reduce sperm production by around 14% [7]. 

One study found that men cycling more than 300 km per week had less than 4% normal sperm morphology - a level associated with poor fertility prospects [8]. If cycling is your main form of exercise and you're trying to conceive, it's worth discussing this with your general practitioner.

Steroids: a hidden fertility risk

Anabolic-androgenic steroids (AAS) - whether used recreationally or to boost gym results - are one of the more underappreciated causes of male fertility problems. These substances interfere with the hormonal signals that tell the testes to produce sperm, sometimes resulting in azoospermia (zero sperm in the ejaculate).

A 2023 systematic review found that AAS abuse accounts for around 2% of male infertility cases [9]. While effects are often reversible, recovery can take over a year after stopping use. A separate meta-analysis confirmed AAS use is detrimental to sperm motility with only a partially reversible negative impact overall [10].

What does "moderate" actually mean?

The World Health Organisation (WHO) recommends at least 150–300 minutes of moderate-intensity aerobic activity per week, plus muscle-strengthening exercise on at least two days [11]. For fertility purposes, this range appears to be the sweet spot - enough to improve sperm quality and hormone balance, without pushing into territory where the body feels chronically stressed.

Brisk walking, jogging, swimming, or moderate gym work all fit. The goal isn't maximum effort - it's a consistent, sustainable level that supports your overall health.

The role of recovery and lifestyle balance

One aspect of exercise that often gets overlooked in male fertility discussions is recovery. While staying active is important, the body does much of its repair and hormone regulation during periods of rest.

Men who train regularly but neglect sleep, recovery days, hydration, or proper nutrition may not experience the full reproductive benefits that exercise can offer. Poor recovery can lead to persistent fatigue, elevated stress hormones, and chronic inflammation, all of which may have a negative impact on sperm production and quality over time. Sleep is particularly important, as testosterone production largely occurs during the night. Research has shown that inadequate sleep is associated with lower testosterone levels and poorer semen parameters. 

Nutrition also plays a supporting role, with nutrients such as zinc, selenium, vitamin D, and antioxidants contributing to healthy sperm development. Rather than focusing solely on workout intensity or calorie burn, men trying to conceive should think about their overall lifestyle pattern.

Consistent exercise, quality sleep, balanced nutrition, stress management, and adequate recovery work together to create an environment that supports reproductive health. In many cases, small sustainable habits maintained over months can have a greater positive effect on fertility than short periods of intense training followed by exhaustion or burnout.
